If you have a child or teenager with diagnosed special needs, and think that your family would benefit from programs offered by our agency, click here to download an application form. Fill it out and send it to us by e-mail, drop it in the mail, or give us a call ahead of time and bring it by the office. Once we've received your application, one of our family coordinators will be in touch to set up a time to meet with you and your child. Please note that if your child has a diagnosis of ASD, we need to receive a referral from your resource coordinator at Contact Hamilton before the registration process can begin for your family.
